Curnyn Physical Therapy is a preferred provider with most major insurance networks and carriers. Some insurance companies still list us under our original name, Sportcare Physical Therapy. It is our policy to confirm the eligibility and benefits of all our patients and inform them of their insurance coverage for physical therapy at the time of their visit. We are also happy to check your benefits prior to scheduling your appointment at your request. Please click here to receive an email containing your co-pay, deductible, co-ins, and/or limitations within your policy. We will do our very best to respond the following business day.
Please provide us with all of the following information for the most accurate response.
We provide insurance billing services as a courtesy for our patients on a weekly basis. Any balance due after the patient’s insurance has paid, such as co-insurance and/or deductible amounts, is billed to the patient on a monthly basis. Co-payments and estimated co-insurance are collected at the time of each visit and patients with annual deductibles that have not been met are asked to pay for their visits until their deducible is met.
We offer a private pay option for those patients who are uninsured or have high deductible plans or high copays. We can also help arrange for 12 month no interest financing if needed. Please feel free to ask for more information!
We accept payments by Cash, Check, Visa/MC debit and credit cards, Amex and Discover Card. We are now also a Care Credit provider which gives our patients another possible alternative to paying for their physical therapy. Care Credit offers affordable financing and even interest free financing to those who qualify. You can get more information and/or apply for credit by calling us or visiting their website www.carecredit.com
Injured workers have 100% coverage for accepted claims through their employer’s Worker’s Compensation insurance. Due to Worker’s Compensation regulations legislated by the State of Texas, an initial evaluation can be done without receiving authorization; however, prior authorization for physical therapy must be obtained before continuing physical therapy. We work with the patient’s doctor and insurance company to obtain this authorization in as timely a manner as possible.
Patients who are involved in a motor vehicle accident may choose to use their Personal Injury Protection (PIP) if it is included in their policy. The amounts generally range from $1,000-20,000 depending on the policy and our clinic can bill the patient’s auto insurance until that benefit is exhausted if necessary. We do not accept 3rd party auto insurance at this time.
